Position Summary:
Provides assessment and crisis intervention; plans, organizes, and implements social work services for patients and families. Provides long term care planning for patients/families. Responsible for complex discharge planning. Intervenes as needed with hospital patients with mental health problems or substance abuse problems. To provide quality care to patients as specified in the individualized care plan and assignments, according to the hospitals’ policies, procedures, and objectives. To provide a safe and clean environment for all patients. Under the supervision of the Program Director and CNO provides assistance with the activities of daily living, physical comfort, and emotional and social support to ensure a holistic approach to patient care. Behaves in a professional manner that is consistent with the mission of Claiborne County Medical Center.
Position Requirements:
A. Licensure/Certification/Registration: Must be licensed as a social worker or certified mental health therapist.
B. Education: Master’s degree in Social Work or Mental Health Counseling required.
C. Experience: Group Therapy experience and two years’ experience working with geriatrics, strongly preferred.
D. Special qualifications: Ability to understand and carry out routine non-technical, oral and written objectives relative to care of patients and or residents; strength and mental alertness; willingness to cooperate and to carefully follow a prescribed routine; discretion; ability to get along well with others and to secure cooperation of patients.

Position Summary: Transport patients of the PHP & IOP program to and from the facility. Behaves in a professional manner that is consistent with the mission of Claiborne County Medical Center.
II. Position Requirements:
Licensure/Certification/Registration: Must have a current MS DL.
Experience: Previous driving and mental health experience.
Special qualifications: Ability to understand and carry out routine non-technical, oral and written objectives relative to care of patients and or residents; strength and mental alertness; willingness to cooperate and to carefully follow a prescribed routine; discretion; ability to get along well with others and to secure cooperation of patients.
